For example, an invented $45 monthly premium costs $540 over twelve months before any separate fee. If a $2,500 bill yields $1,600 of reimbursement, the retained invoice is $900 and combined annual spending is $1,440. The clinic may still require the full $2,500 initially. These numbers are arithmetic illustrations, not market estimates or quoted prices.
